Coastal Tanker Engine Room Zonal Vulnerability Analysis (251-01)
Fire in unmanned engine rooms of ships are a major concern. R2A was commissioned to inspect the engine rooms and review the potential problems and solutions in a workshop. A zonal vulnerability approach was adopted to determine if ships were at risk from various threat scenarios. A generative risk based approach consistent with James Reason's ides was used to develop optimal solutions. The project was completed with Allan Kneller & Associates.
Coode Island Access Modification (318-09)
A workshop was facilitated to identify and document the issues and possible solutions associated with planning, design, construction, operation and maintenance of the proposed modification works as they effect third party stakeholders.
Deep Draft Vessel Survey (396-01)
R2A undertook a deep draft vessel survey of the Port of Dampier to ensure all credible critical issues had been identified and appropriately addressed. This was completed using a common law safety case approach.
Fast Ferry Wash Risk Analysis (240-01)
The Marlborough Sounds, South Island, NZ, are a major tourist attraction. They are also part of the ferry route between the two islands. The interaction between the ferries and small vessels was the subject of the analysis. The risk analysis was commissioned by the Harbourmaster. The project was completed in association with Kerry Dwyer & Associates.
IMO Formal Safety Assessment (343-01)
A Formal Safety Assessment was undertaken of South Port, NZ's pilotage services and supporting marine infrastructure. The assessment was consistent with IMO guidelines and AS/NZS 4360:2000 and extended to include generative best practice approaches as espoused by James Reason and others.
Independent Risk Review at Marstel Terminals (318-09)
A review of the existing Marstel Terminals Coode Island risk assessments was undertaken, and risk advice provided on the proposed locations of the new PoMC facility gatehouse and MTCI offices, and on the existing gatehouse and P&O Ports Offices.
Pilotage Reviews
A number of pilotages reviews have been completed for various Australian Ports including Brisbane, Geelong, Hobart, Devonport, Burnie and Launceston.
Management of Recreational Boating on Port Water (318-22)
This was a review of the implication of the current jurisdictional arrangements with regard to recreational boating safety in Port waters. The objective was to clearly identify the roles and responsibilities of the relevant agencies.
Maribyrnong Berth Fire Design Review (318-06)
A risk review of the design contract for the upgrade of the fire fighting system at No. 1 Maribyrnong berth.
MRS Pioneer Explosion Risk Review (147-01)
Independent review of explosion and deflagration potentials and controls as identified by others on board the bulk sugar ship MRS Pioneer
Risk Management Reporting (318-10)
A briefing paper presenting various risk management reporting techniques was prepared for discussion with the Port of Melbourne Corporation. The aim was to select a technique appropriate for reporting issues to the PoMC Board.
Strategic Risk Reviews (318-01 to 318-05)
Various high-level strategic risk reviews using a threat and vulnerability approach have been completed for a number of departments within the MPC Infrastructure Division.
Transfer of Station Pier (318-11)
A workshop was facilitated to identify and prioritise the key issues associated with the transfer, operation and ongoing maintenance of Station Pier and to document possible mitigation options.
