Digital Voice Announcement & Traction Interlocking
In order to produce the safety case argument for the installation of digital voice annoucement and traction interlocking, a number of different risk studies were completed including a high level project vulnerability assessment and risk workshops with relevant stakeholders for each stage of the project including trial implementation and roll-out. Reliability modelling of the final design was also completed as well as cause-consequence modelling and threat barrier diagrams for the overall caught and drag risk.

Dynon Port Rail Link
Stakeholder workshop to identify and assess any construction and eventual operational risks associated with the track and signalling works. The results fed into the final construction tender documents.

Electrical Network Risk Review (365-05)
A high level review of the rail electrical network was undertaken to comprehensively identify internal and external risks that have the potential to affect the safety and/or reliability of rail services in the Sydney metropolitan area.

Footscray Road Level Crossing (115-23)
Event tree modelling based on the existing controls was developed and defines the loss of control point as loss of vehicle kinetic energy, which has the potential to result in a train/vehicle collision.

Grovedale Station (115-28)
Two issues Workshops were held to identify and document they key issues and appropriate precautions associated with the introduction of the new station at Marshalltown Road, Grovedale.

Lapstone Cutting Due Diligence Review (365-20)
A due dilligence review of RailCorp train operations through Lapstone Cutting was completed to ensure all sensible praticable precautions are in place to address issues associated with rock falls in the area.

Lynbrook Level Crossing (296-01)
This was an expert witness case regarding the introduction of new level crossings in the community.

Lydiard Street Gates, Ballarat (173-01)
A vulnerability assessment to identify the critically exposed groups at the crossing and the potential threats.

Melbourne Airport Rail Link (252-01)
This analysis showed that the 7 Broadmeadows level crossings were the most problematic risks in the comparison between the Albion and Broadmeadows corridors.

Menangle Bridge Risk Assessment (212-22)
A risk assessment to outline the actions and precautions that should be in place for reopening the Menangle Bridge.

New Street Level Crossing Due Diligence Review
This review documented the safety issues associated with the manually operated level crossing gate nd identify generic control options for consideration in order for Connex to discharge its statutory and common law obligations.

On Track Vehicle Operator Incapacitation (365-04)
Track vehicle driver incapacitation review. Appropriate precautions recommended to help justify that the drivers of some or all vehicle types could be categorised as Category 2 workers for medical assessment purposes.

Regional Fast Rail (371-03 - 371-07 & 346-02)
A number of risk reviews were completed for the RFR Project including a review of all level crossings on the four corridors, implementation of TPWS, Safeworking Rules and a high-level independent safety review.

Role of the Outer Handsignaller (365-01)
This risk assessment was to complete a desk-top review of the role of the outer handsignaller task and assess the situation where the outer handsignaller becomes incapacitated and is not performing his duty i.e. the train driver is not warned of an approaching worksite and continues to travel at line speed until he reaches the inner handsignaller.

Strategic Risk Review
A high-level strategic risk assessment of the Australian Rail Transport Corporation (ARTC) proposal for the rail network in NSW. The risk assessment comprised a series of generative interviews and a risk assessment workshop.

Trespasser Death Analysis (212-01)
A review of trespasser deaths on NSW railway property was conducted based on historical data to determine the adequacy of the controls and precautions in place i.e. fencing, signage, etc.
